SAIL Insurance Payment
The cost of insurance will be $5.00 per swimmer. This is the same cost as several pervious years. You must pay for Coaches and Guppies as well. Please include these along with your other swimmers. Payment for your swimmers is due at the first SAIL rep meeting in May (May 1) with a typed list by pool name and the following information:
- Swimmers Name (first, middle and last)
- Address of swimmer (optional if using the sort from Team Software)
- Date of birth
- Swimmers by Age Group indicating Gender (no need to split between girls and boys) 8 & under, 9-10, 11-12, 13-14 and 15-18
You may prepare the listing from Team Manager (see instructions below). Insurance is due for all swimmers, including guppies (even if they change their mind after only one practice), and all coaches.
For swimmers that sign up after the first SAIL rep meeting in May, please e-mail those names with the requested information to the e-mail address given below. Please bring the insurance payment and written list for the additional swimmers to the subsequent SAIL meetings on May 30th and June 26th. The final report with any additional premium will be sent to the Insurance carrier after the June 26th meeting.
It is the responsibility of the SAIL rep to obtain and retain a Wavier and Release of Liability (click here) for each swimmer. The SAIL rep must also prepare and sign a Waiver and Liablity Affidavit (click here) which must be given with the insurance information to the Insurance Chairperson.

Insurance Report Instructions Using Team Manager
III. Team Roster and Insurance Reports
1. After entering data for new swimmers and editing existing swimmer’s data, Print a Team Roster and double-check the swimmer names, birth dates, and practice groups, and any additional information against the registration forms.
- Select {Print Reports}{Administrative}{Roster, Labels, and Attendance}
- Select the Practice Group, Age Groups, Gender, and Roster Format
- Print/Preview
- Review on screen, or print to printer
2. Insurance Report: your SAIL Rep will eventually ask for this in May.
- Select {Print Reports}{Administrative}{Rosters, Labels, and Attendance}
- When the window appears select the following report criteria:
Sort By – Birth date
Roster With Address
Both Male and Female
Practice Group – Choose ALL for your whole Team
Age Group – ALL - After making the above selections click on the Print/Preview button
- When the preview of the report shows, click on the print button to print the report
- Your report will have two lines per swimmer. It should be sorted with the oldest swimmers first
- Using a ruler and pen draw lines across the report between the age groups. Notice they are NOT the usual SAIL age group designations: 16-18, 13-15, 12 & under. These ages are for insurance purposes only.
- Manually add any coaches that are not swimming to the report so that they will be insured.
